Compassionate, Parental and Emergency Leave Policy

Policy outlines what constitutes compassionate, parental, dependents, family emergency, treatment and garden leave, and the procedures to follow when requesting, taking, authorising and declining such leave.

Annual Leave Request & Absence Record Forms

Three forms to help employees and line managers:

  • request annual and other sorts of leave
  • provide sufficient information about time off work
  • authorise requests (and note whether paid or unpaid)
  • log time off from work during an anual leave year
  • highlight and spot patterns (such as sickness, lateness)

Form 1 - for staff to request leave and for managers to authorise leave
Form 2 - to record leave requests, sick leave, timekeeping and attendance
Form 3 - monthly departmental absence report

Using these forms helps managers to monitor staff absences, manage time keeping, identify and address attendance problems at an early stage and ensure employees take their statutory leave entitlements.

(Three Absence Record Forms are included as standard in this pack: one for January to December annual leave year; and one for an April to March annual leave year.)
